- By purchasing counseling, coaching or therapy services, you are giving us permission to use your personal data anonymously with no identifiable information provided for research and scholarly publication, or supervision purposes.

If you purchase our Counseling/Therapy services
We cannot guarantee symptom reduction or resolution of issues addressed or not addressed in counseling/therapy sessions. We are responsible for providing the counseling/therapy services purchased and agreed to in the informed consent agreement. These services will be delivered by Dr. Rachael Haskell in accordance with ethical guidelines and policies and procedures set forth by the National Association of Social Workers Code of Ethics (1996) and the Florida Board of Clinical Social Work, Marriage & Family Therapy, and Mental Health Counseling.
We ask that you please notify the office at least 24 hours in advance should you need to cancel/reschedule your appointment. If you notify us within 24 hours you will not be charged for the session, and we will be happy to reschedule your appointment at a time convenient for you. However, if you fail to notify us within 24 hours, we will charge a $25 fee.
Therapy sessions are confidential unless you present an imminent, serious danger of violence to another or yourself; or if a Judge issues an order mandating that client records be released or information therein. In these cases, necessary information may be released for legal or safety reasons. Another circumstance where protected health information may be released is if you or your guardian make a request that your information be shared with another whom you specify in a signed release of information form.
If you participate in our Educational/Training Courses
By enrolling and participating in any training course offered by Dr. Rachael Haskell, you agree that she only represents that she will provide you with the material and information necessary to satisfy the course requirements for any of the certifications offered by Dr. Rachael Haskell, and for the Continuing Education credits for the FL Boards of which Dr. Haskell is a provider. This is not a guarantee that you will pass the legal requirements of the professional board in the state of FL. You must ensure you that you meet all other licensing requirements to qualify to become licensed, or maintain licensure. Your failure to fulfill any licensing or certification requirement is beyond the control, fault or cause of Dr. Rachael Haskell. You agree to fully release and waive any and all claims against Dr. Rachael Haskell relating to your failure to become licensed or to pass any of the requirements to become a Certified Hypnotherapist or Approved Licensure Supervisor, or to meet the requirements of your job or ethical code of your helping profession.
Our training material is intended for the sole purpose of providing helping professionals with continuing education. By using our website to purchase training, you are giving us permission to share recordings of our training on YouTube and other social media outlets. Dr. Rachael Haskell is not responsible for any unethical or potentially harmful use of this information. Upon purchase of a training file, it may be downloaded a maximum of 5 times before it expires. Upon completion of a course, continuing education credits will be submitted to CEbroker.com within 2 business days.
If you are not satisfied with the training, your full fee will be refunded if requested prior to the conclusion of the training, and no continuing education credits have been issued.
DISCLAIMER Regarding Exam Preparation material- Dr. Rachael Haskell is not affiliated with the Association of Social Work Boards, National Board of Certified Counselors, Association of Marital and Family Therapy Regulatory Boards or the panels that created, manages, or scores the scenarios for their respective exams. There is no communication between these bodies regarding the format of the exams or prior information shared by those boards to Dr. Haskell. In addition, all material is paraphrased where the DSM-5™ and ASWB, NBCC, or AMFTRB information is contained within our exam preparation material. Materials contained within this supplement relative to the DSM-5™ are paraphrased or credit is applied. Practice exams are designed according to a Practice Format similar to the design utilized by the ASWB, NBCC, & AMFTRB on their respective national licensure exams.
Refund and Cancellation Policy:
Cancellations are provided with full refund within 60 days of payment if cancellation is received prior to the program the participant is registered for. If the cancellation is past 60 days a 90% refund will be applied when cancellation is received prior to the program the participant is registered for. A participants tuition/registration can be moved to any future training if participant prefers a different date.
Participation in our training is restricted to licensed mental health and medical professionals, registered interns, undergraduate and graduate students in mental health, psychology, social work, therapy, and medical training programs.
If you purchase our CE Sponsor Services
We will be responsible for submission of the Continuing Education hours for credits earned with the *FL DOH 491 Board for attendees of the sponsored training. We will submit these CE's for approval within 2 business days of receiving a copy of the completed Sign-in Sheet & Training Evaluations completed by attendees from you at our MAILING ADDRESS. We are not responsible for providing the sponsored training, or responding to any complaints received for the sponsored training unless it is regarding the submission of CE's. We will forward any grievances that need your attention to the contact person you list on the CE Sponsor form within two business days. You are responsible for responding to any grievances related to the sponsored training within two business days of receiving the complaint. We will provide Templates for Sign-in Sheets, Training Evaluation, and Certificate of Completion for your use upon purchase. By purchasing this service, you are giving Resolution Today permission to use licensure and evaluation data provided by attendees to obtain CE approval with the FL Board of Clinical Social Work, Marriage and Family Therapy & Mental Health Counseling.
If you purchase a Training or Job Announcement from us
Your advertisement / announcement will expire after 30 days. If you would like to renew your advertisement / announcement for an additional 30 days and keep the same wording as your previous one, place your order 7 business days before its expiration.
We are not responsible for recruiting any job applicants or training participants in exchange for your purchase. We are not responsible for the content of the paid advertisements / announcements placed on our website.
We may revise these Terms & Conditions. The revised Terms & Conditions will apply to the use of our website from the date of the publication of the revised Disclaimer on our website.
Law and jurisdiction
These Terms & Conditions will be governed by and construed in accordance with Florida law, and any disputes relating to these Terms & Conditions will be subject to the jurisdiction of the courts of Pinellas County, Florida.
Registrations and authorizations
Dr. Rachael Haskell is licensed and governed by the Department of Health, Division of Medical Quality Assurance, for the State of Florida.
